The contact form asks for your name, phone number, email address, and a message. Please provide general questions and contact information only. Do not submit medical records, Social Security numbers, insurance numbers, or other sensitive information.
How your inquiry is handled
Submitting the form sends your inquiry through Formspree, a third-party form service, so Birdie’s can respond about senior living guidance. Birdie’s uses the information you provide to contact you and discuss your request.
Third-party services
This site is hosted on GitHub Pages. Fonts may be requested from Google Fonts, and form submissions are processed by Formspree. These providers may process technical information, such as IP addresses, under their own policies. Links to Facebook or other sites take you to services with separate privacy practices.
Discussing information with care providers
The website form does not submit your inquiry directly to a care community. Before supplying information for a referral, discuss with Birdie’s which provider will receive it and what information is needed. Do not send another person’s information without appropriate permission.
Your questions and requests
To ask about information you have submitted, request a correction or deletion, or ask us to stop contacting you, call Tricia at 806-252-8877 or Sarah at 806-928-5880. Explain your request without supplying additional sensitive information. Retention requirements and third-party processing may affect what can be deleted.
